How to Submit Your Nominations

We are accepting nominations for the 2008 Welcome Back Awards – the 10th anniversary of this exciting awards program! To nominate yourself or someone else for one of the five awards, please do the following.

Important Note: You must have Acrobat Reader version 4.0 or higher installed to view these files. If you have an earlier version installed, an update is available from the Adobe Acrobat Reader download site.

Please clearly print or type your submission. Be sure to include your (and your nominee's) name, address and telephone number.

For full consideration, please be sure you include the following information in your nomination packet:

• Nominee biography (no more than 2 pages); nominee must be over 18 years old
• At least one letter of support (no more than 3)
• A one-page personal statement by the nominee

Make certain that your nomination specifically addresses the criteria outlined in the award category descriptions. Nominees are encouraged to provide examples of their accomplishments (e.g., what makes them, or their approach to treating depression, unique; how their efforts have influenced others).

Please note: Submissions must address the nominee's accomplishments as they relate to clinical depression. The Awards Committee assumes that mental health consumers who are nominated have done so with their full knowledge and consent.
