Medication Safety Tools

The Institute of Medicine (IOM) report To Err Is Human: Building a Safer Health System (IOM, 2000) identified medication errors as the most common type of error in health care and attributed several thousand deaths to medication-related events.

Regardless of whether one considers errors of commission or omission, error rates for various steps in the medication-use process, adverse drug event rates in various care settings, or estimates of the economic impact of drug-related morbidity and mortality, it is clear that medication safety represents a serious cause of concern for both health care providers and patients. -Preventing Medication Errors, Institute of Medicine, 2007
