Report a Possible Side Effect or Product Quality Complaint

We encourage healthcare professionals, patients and caregivers to report any side effects or problems with your Lilly medicine to help us ensure our products remain safe and effective.

If you experience a side effect while using a Lilly product (medicine or device), you should always seek medical advice from your doctor.

If you have concerns about a possible side effect, the quality, usability, appearance or safety of a Lilly medicine, please share your experience using one of the options below.

How to Report

If you live in the United States, the primary method to report side effects or quality complaints is through this online tool.

What to Expect

Provide information to complete your report including, but not limited to:

  • Contact information

  • Age

  • Product Name and Dosing Information

  • Batch/Lot Number

  • Date you experienced the side effect or quality concern

  • Description of the side effect or quality concern

  • For quality concerns, if the medical device is in your possession (please retain it for 14 days)

Submit the report:

Once successfully submitted, you will receive a confirmation number, which lets you know that we have received your information.

Due to high volumes, it may take some time for us to evaluate each report. Please note that we will only reach out to you if we need any further information. If we have all needed information, you will not be contacted by us.

Please do not call to follow up on your submission; this will not speed up the process and may delay our ability to process your submission efficiently. If you have a new side effect or quality concern, please submit a new report.

Follow-up (if applicable):

If permitted, and only as required for product quality concerns, a representative from Lilly may contact you within 3-5 business days to gather additional information, using the communication channel you selected in the online tool.

Important Note:

This online tool is specifically designed for reporting side effects or quality concerns. Using it for other purposes may not reach the appropriate party for the desired response (e.g., this tool is not used to request or receive a product replacement).
